| NOTICE: Due to the "On-Line Filing" application being under construction, a zero report can be e-mailed to the Department by providing the holder number, report year, FEID number, name of company, mailing address, city, state, zip code, contact person, telephone number, e-mail address, and a statement that this is a "Zero" or "Negative" report. - Reporting Instructions
- Outlines the duties and responsibilities of a holder in reporting unclaimed property. Included are the required forms for paper reporting, electronic reporting instructions, Florida's dormancy period table, etc. NOTICE: Section 717.117(1), Florida Statutes, requires that a report identifying 25 or more different apparent owners must be submitted by the holder via electronic medium (diskette or CD-ROM). In addition, the Bureau of Unclaimed Property requires that all electronic reports be submitted in the NAUPA format revision which became effective November 2003.

- Paper Reporting Forms
- For a holder reporting less than 25 apparent owners, the paper reporting forms may be used. Paper reports must be filed on the Department's forms. Generic forms are not accepted. The information should be typed on to the forms.
